For faxing, we suggest a font size of at least 12 points. You may want to type in the quantities, amounts and order information before printing the form. If you are writing your information, please use a pen and print in block capital letters. After selecting the titles you want to order, you may want to delete the lines for the titles not ordered, these instructions and other superfluous bits, which shortens the form a bit. You may also want to save the modified file under a different name. Print the completed order form. Sign it before mailing or faxing. Credit card orders cannot be processed without the card holder's signature. Shipping time will vary depending on your method of payment. The longest delays can be expected when mailing a check. It may take a week or two to reach us and another two weeks or more (10-15 business days - the banks' estimate, not ours) to clear at the bank. That could mean about five weeks before your order can be shipped. So please, be patient.
